September 2026 – Onpoint is proud to have partnered with the Connecticut Office of Health Strategy (OHS) on their recently launched Connecticut Health Equity Dashboards, an interactive tool that brings together multiple data sources to allow users to explore health outcomes, healthcare access and utilization, and the social factors that influence health.

The dashboards integrate standard health metrics – for example, the rates of avoidable ED visits, appropriate cancer screenings, and well child visits – derived from data in the state’s all-payer claims database (APCD) with other state and national data sources, including the Social Vulnerability Index (SVI) and the Area Deprivation Index (ADI). This gives users – whether providers, community organizations, policymakers, or Connecticut residents – the ability to explore how income, housing costs, access to transportation, and other community conditions across Connecticut are associated with chronic disease burden, quality of care, healthcare utilization, and overall health outcomes.

Developed with federal American Rescue Plan Act (ARPA) funding and guidance from a multi-agency design group, the dashboards are meant to support data-driven decision making and identify opportunities to advance health equity across Connecticut.
